Indemnification Forms
All vendors, exhibitors, restaurants that set up at the ArtsPark on 4/28 must complete both indemnification forms using blue ink. Forms must be mailed or dropped off at the Chamber offices prior to April 23, only original copies accepted, no faxes or photo copies. Please write "Attn: Expo Al Fresco" on envelope.
